
The Pod Model of Care

A New Approach to Community Care
IWC is changing the way we provide care to our community to allow for better access; one way in which we are doing this is through the new pod model.
The new Pod model is based on our ways of Respect (for your story), Connection (with you and your family), and Kinship (Ways of working together).

What is the pod model?
This new way of care is centered around strong, healthy relationships between you and staff through trust and cultural safety.
- A Pod is made up of a small team of health providers that will look after you and your family at every visit to IWC.
- Your Pod gets to know your story and work closely together to support you on your health journey.
- This means stronger relationships between you and your health providers, culturally safe care and better support for you and your family

Who's in the pod?
Pod teams all work towards what you and your family need to be healthy, strong, and deadly. Each Pod Team includes a:
- Aboriginal Health Worker
- Receptionist/Administration
- Nurse
- Doctor (GP)
- Aboriginal Health Practitioner
Depending on your needs, the core Pod team members will also be able to connect you and your family with dental, allied health services and a range of other health professionals available at IWC.
Pod team roles
Aboriginal Health Worker/Practitioner
Works closely with you and your family, alongside the rest of your Pod team. They help you understand your health, offer support, and walk with you on your health journey to reach your goals.
Administration / Reception
Helps keep everything running smoothly. They’ll connect you with your Pod team, book your appointments, help organise transport if needed, and supporting you with paperwork like Medicare.
Nurse
Keeps track of what’s important for your health, shares key info with the GP, and helps you and your family follow your care plan.
Doctor (GP)
Supports your overall health and wellbeing. They’re here to listen, give medical advice, and care for you when you’re feeling unwell or need help with your health.
