"Brokerage Service"
The "Brokerage Service" is coordinated by the IWC and is primarily based around providing diabetes related care to ATSI patient/clients with or at risk of diabetes. The service also covers other "high priority" health issues when all other avenues of health care assistance (eg EPC, Mass, DVA) have been exhausted.
Who can Refer to the Service?
Only General Practitioners and or Allied Health Professionals can refer patients/clients under the Brokerage Services Model. Referrals should include relevant information, recent blood tests, medication list and or reports.
Who can Access the Brokerage Service?
ATSI patients/clients with or at risk of diabetes. Other "high priority health issues will be considered where all other avenues of assistance have been exhausted.
What is the Approval Process for Brokerage Referrals?
The clinical Nurse monitors all external Brokerage referrals. Approval is based upon a clinical/managerial/financial basis. Once approved IWC will make linkages to purchase services.
